Student Health Benefit Information
If you are looking for information concerning your student health benefit, need your benefit card, or would like to track your claims this year, please remember to log into your account at www.studentinsurance.com.  There you can find all of your benefit information as well as be able to print your benefit card. You will need your Student ID number (not your blind grade number!!) to initially login.  You can find your Student ID on the Student Services Portal, usually on your student account. If you have any questions, please feel free to contact Assistant Dean Harris for assistance.

Room Reservations
To reserve a room, please complete and submit an Event Approval Form. Included in the Event Approval Form (a form that must be completed and submitted before a student organization can sponsor an event on campus) is a section pertaining to room reservations, which asks you to specify, among other things, the following: proposed date and time of the event, alternate date and time for the event, and expected number of attendees.

Copies of the completed Event Approval Form should be submitted to the Student Services Office at least two weeks in advance of your scheduled event. Requests for room reservations will be addressed in the order in which they are received. You will receive an email once your request has been approved and the space has been reserved.

If your event requires setup of any kind (i.e., A/V needs, food, special setup from Buildings and Grounds), please be sure to address those needs in the Event Approval Form. The Event Approval form can be picked up from the Student Services Office and contains a section pertaining to A/V and other support. Upon receipt of the form, the A/V staff will be contacted to determine their availability and make the necessary arrangements.

If your event is taking place off campus and involves alcohol, sports or other physical activity, your participants must sign a liability waiver form. Please see Student Services for copies of this form.
